Grant Regional Health Center (GRHC) is deeply rooted in its surrounding communities, and provides many established programs and services to support community health. GRHC has once again collaborated with a number of area agencies to conduct a Community Health Needs Assessment (CHNA) in compliance with IRS 501(r) guidelines. Our partners include Gundersen Boscobel Area Hospital and Clinics, Crossing Rivers Health, UW Extension, Grant County and Crawford County Public Health Departments, Aging and Disability Resource Center, and others.
A CHNA provides crucial information about the health and wellness needs of the communities we serve. As part of the CHNA, we asked from stakeholders like you to take a short survey as a glimpse into current health issues and trends in both Crawford and Grant Counties, and the neighboring areas. This input is an integral part of the assessment process.
