Grant Regional Health Center is once again collaborating with a number of area agencies to conduct a Community Health Needs Assessment (CHNA). Our partners include Gundersen Boscobel Area Hospital and Clinics, Crossing Rivers Health, UW Extension, Grant County and Crawford County Public Health Departments, Aging and Disability Resource Center, and many others.

A CHNA provides crucial information about the health and wellness needs of the communities we serve. We’ve created a short survey to gather insight, from stakeholders like you, into current health issues and trends in both Crawford and Grant Counties, and the neighboring area. Your input is an integral part of the assessment process.

Here’s how you can help: Please spend approximately 15 minutes of your time to complete a survey, sharing your perspective on current health issues, resources and trends affecting communities in Clayton, Crawford and Grant Counties and the surrounding counties.
