Dr. David Casillas Plazola, Family Medicine/Obstetrics
Dr. Casillas Plazola specializes in Family Medicine and Obstetrics and is excited to make Lancaster his home. He looks forward to practicing full spectrum family medicine including obstetrics while developing long term relationships with his patients and their families. He completed his medical degree at the Medical College of Wisconsin-Milwaukee and his residency in the Fox Valley Family Medicine Residency Program. He is fluent in Spanish and English. “My goal is to empower patients with the medical advice, screenings, and resources to help achieve their best health outcomes. I believe in taking the time with patients to explain their health issues and the strategies we have to prevent complications and remain as healthy as possible,” explains Dr. Casillas Plazola. Dr. Dale Chilson, Board Certified Ear, Nose, Throat & Facial Plastic Surgeon
Grant Regional is excited to offer Otolaryngology/ENT services locally, so patients won’t have to travel great distance or wait to be seen to receive this important service for their family.
Dr. Chilson has over 27 years of experience providing both surgical and minimally invasive treatment options for the ears, nose, and throat to ensure positive outcomes with minimal down time.
We offer a very wide array of ENT services for prevention, diagnosis and treatment of children and adults including:
- Sinus infections
- Chronic allergies
- Frequent respiratory illnesses
- Balloon sinuplasty
- Removal of facial cancers/lesions and reconstructive surgery
- Adenoid and tonsil removal
- Ear surgery (ear tubes, repair holes in ear drum, otosclerosis)
- Hoarseness, swallowing disorders and throat cancer screening
Dr. Chilson works closely with Grant Regional’s audiologist to provide comprehensive, state-of-the-art audiological testing for patients of all ages including:
- Adult and pediatric evaluations
- Hearing aids and assistive listening devices
- Balance disorders (including vertigo and dizziness)
- Tinnitus evaluation and therapy

Rachel Loeffelholz, MSN, APNP, FNP-BC, Family Medicine
Grant Regional is also excited to welcome Rachel Loeffelholz to the new role of Family Nurse Practitioner. Rachel has been an RN with Grant Regional Health Center since 2016 and recently graduated with her Master of Science in Nursing - Family Nurse Practitioner (MSN-FNP).
"After spending many years as an RN in a variety of settings, I understand the importance of listening to my patients. Creating therapeutic trusting relationships is a key component of my practice. I believe it is important to acknowledge each person’s individuality while providing compassionate care, educating patients and families, and striving to promote healthy habits/ preventative care for individuals of all ages."
In addition to caring for patients at Grant Regional, Rachel also proudly serves as a volunteer EMT with the Potosi EMS. In her free time she enjoys spending time outdoors including hunting, fishing, or hiking. She also keeps busy with her family farm.
