To access the clinic, turn off of Hwy 18 to Bronson Blvd, and take a right onto Lafollette St. We are located across from the ballpark, just past Fennimore Smiles!

We welcome new and existing Grant Regional Patients. Appointments can be made with any of our Grant Regional Community Clinic - Fennimore Providers by calling (608) 723-2131. In addition to all of our physicians offering primary care, Dr. David Casillas will also see Obstetrics patients. Our team will also offer in house X-ray imaging, Rehabilitation/Physical Therapy, and Lab services on-site.

Carrie Barker, CMA, recently joined the GRHC team in October 2023. Carrie has been working with provider Brian Quick, PA-C, for more than five years, and has been a Medical Assistant for nearly 27 years. Over the years she has mostly worked in Family Practice but has also worked in OB/GYN, occupational medicine, podiatry, and pediatrics. Carrie resides in Fennimore and has two sons, Cash (15) and Conrad (13). When not at work she and the boys like to spend time on their family farm just outside of Mineral Point. Carrie is enjoying the perks of working close to home at the new Fennimore Clinic and continuing to work with Brian Quick. Carrie shared, “Brian is very passionate about taking care of patients and providing the care that they need. I am very happy to be part of this amazing team in Fennimore providing care in this community and surrounding area.”

Samantha Stagman, CMA, joined the Grant Regional team last September. Samantha and her four siblings grew up near Patch Grove and attended River Ridge High School. In May 2023, Samantha graduated from Southwest Technical College. Samantha loves animals and has three dogs, two cats, and a goldfish she won at the Grant County Fair over ten years ago. She also loves playing volleyball, ATV riding, and camping. Samantha enjoys working alongside the nursing team at the Fennimore Clinic and seeing new and familiar faces. Samantha works primarily with Dr. Ron White and shares, “Dr. White is very professional and is very dedicated in the work he does. He likes to be productive and get work done.”

Brynn Junk, RN, also joined the GRHC team in October of last year. Brynn began her career in the medical field as a Medical Assistant in 2006. She worked full-time and went to school at Southwest Technical College to become an RN in 2013. She shared, “I love what I do, and I’m still learning something new every day.” Brynn and her two children, Aven (18) and Jonathan (13), enjoy country living just outside of Fennimore. Working at the new Fennimore Clinic, Brynn is excited to continue serving patients she has grown to know over the years as well as meeting new patients. Brynn has worked with Brian Quick, PA-C for 17 years, and will also be working with Dr. David Casillas Plazola and Dr. Ron White. Brynn added, “We have a great team in Fennimore and will continue to provide excellent care for the community.”

Cassondra Gehrke, CPC, CMA, has been with Grant Regional since early 2022. Cassondra originally began with a degree in medical coding, working in chart abstraction, all the while she was furthering her education to become a CMA. Cassondra is very family oriented; she likes to spend as much time as possible with her significant other and her two boys outside of work. You can often find her relaxing by reading a good book, assembling puzzles or creating art. Cassondra is excited about and enjoying the new environment of the Fennimore Clinic. Cassondra works primarily with Dr. David Casillas-Plazola and shares, “He is an amazing, caring, attentive, intuitive provider. He is very invested in his work and his patients. I am very lucky to be there helping him build his practice.”

TC Streif, RN, has been a member of the Grant Regional team since 2018. TC started her career in the medical field as a CMA and later attended Southwest Tech to become an RN. TC has two adult children and 1 dog. Her favorite thing to do outside of work is to enjoy a good campfire with friends in the spring and fall months. TC grew up in Fennimore and looks forward to helping the community she grew up in. TC works with all of the providers at the clinic and enjoys talking and seeing many people throughout her day.

Faces of Fennimore - Physical Therapy Team

Brandon Krantz, DPT, joined the Grant Regional Health Center team in May of 2023. Brandon was born and raised in Fennimore with his older sisters Allie and Sam. During college he coached the middle school football team and currently helps to coach the high school football team. Go Golden Eagles!

Even as one of the newest members of our team, he’s undergone extensive training in many different settings and patient populations. He has completed rotations in acute physical therapy that consisted of work in an Emergency Room, ICU, and Neuro Unit; a sports Physical Therapy rotation that included working with college athletes and research on running mechanics; and several other Outpatient PT rotations.

Brandon looks forward to giving back to Fennimore by helping community members get back to doing the things that they enjoy! When you see Brandon outside of work, he might be playing or watching sports, spending time with friends, kayaking or walking his beloved dog, Scout!

Whitney Zink, DPT, has been working as a Physical Therapist for more than 11 years. Prior to joining the Grant Regional Health Center team, Whitney worked in a skilled nursing facility as a Physical Therapist and Rehab Manager in Boscobel and Prairie du Chien. Whitney joined our team PRN in 2021, but became a full time employee in December 2023. Whitney has a strong knowledge base in geriatrics, and is Lymphedema Certified.

Similar to Brandon, Whitney grew up in Fennimore and still resides here with her husband, Chuck, and their three children - Brylie, Isabelle and Jameson. In her free time Whitney can often be found golfing or traveling with her family, or coaching her children in softball or volleyball. For the past six years Whitney has enjoyed coaching summer recreation softball league.

Whitney is excited for the opportunity to see patients at the Fennimore Clinic, offering services to the folks she grew up with in Fennimore that were not previously available in the community!

Lindsey Halverson, DPT, has been a Physical Therapist at GRHC for nearly ten years. Lindsey primarily works with outpatient Physical Therapy patients, and also assists in areas of school based physical therapy, inpatient and home health. Lindsey specializes in the areas of orthopedics, pediatrics, vestibular, dry needling, and ASTYM (soft tissue therapy). Lindsey enjoys working in a variety of settings with patients of different ages and diagnoses.

Lindsey grew up in the Montfort area and still resides there with her husband and five busy children. Lindsey has already met patients with ties to the family and friends she has in the area and looks forward to continuing to develop a good rapport with the community members she treats.

Lindsey shares, “I am most excited to work in Fennimore to help serve not only the Fennimore residents but those the outlying community with closer health care to limit driving time and increase access to not only physical therapy services but imaging, lab and medical as well.”

Faces of Fennimore - Radiology & Lab Team

Sarah Torgeson, Radiologic Technologist R.T.(R) & Breast Care Coordinator, joined Grant Regional Health Center in 2019. Sarah grew up in Fennimore and still resides just outside of town. She looks forward to seeing patients she knows from the community. Sarah is experienced in both lab and radiology capabilities as she previously held a similar role before joining GRHC. Sarah also runs a small craft business in her free time! She loves to travel and spend time with her family including her boyfriend Jason and 3 kids - Mariah (24), Ashley (23), and Drew (14). She is eager to add the title of first-time grandma in July of 2024!

Lynlee Udelhofen R.T.(R)(CT) has been a member of the Grant Regional team since 2017. Lynlee lives in Fennimore with her husband and their daughter, Lennon, along with two dogs, Riley and Lucy. Outside of work they love to enjoy the great outdoors by kayaking, fishing, shed hunting, or enjoying a nice bonfire with friends and family. Lynlee is excited to have a part in offering x-ray and lab services to the Fennimore community! Lynlee has enjoyed upping her lab skills in this new environment and appreciates the wonderful co-workers alongside her at the Fennimore Clinic who have been very helpful as well as Sam Brooks, GRHC Laboratory Manager.

Bridgette Myers, Radiologic Technologist R.T.(R), has been a member of the Grant Regional Health Center Team since June 2021 and is excited to now work with the amazing clinical staff/providers at Grant Regional Community Clinic – Fennimore, and to be able to offer both Radiology and Lab services in the clinic setting! Bridgette grew up on a dairy farm with three siblings, which nurtured her love for the outdoors. She enjoys staying busy on the farm with her fiancé and helping raise beef cattle. She also has a passion for horses and enjoys riding and barrel racing as much as possible!

Brian Quick, PA-C, is a Family Medicine provider with over 15 years of experience in our local area. Brian is passionate about working alongside patients to develop a plan for their health, together. Brian and his team have a patient-first attitude and he looks forward to continuing to practice medicine in the area where he grew up. Brian will see patients at the Fennimore Clinic on Monday, Wednesday, Thursday, and Friday.

Ron White, DO, has recently joined the Grant Regional Health Center & Grant Regional Community Clinic team. Ron grew up minutes away from Lancaster in the small community of Mt. Hope, WI. After being away for 13 years, he is thrilled to get back to his roots, settling down in the same area he was raised with his growing family. David Casillas Plazola, MD, is an Obstetrics & Family Medicine Provider. Dr. Casillas was born in Guadalajara, Jalisco, Mexico. As a teenager, he moved to the central valley of California with his family where he completed his Bachelor of Science in Human Biology. After undergrad, he sought out an adventure in the north to pursue his medical education at the Medical College of Wisconsin in Milwaukee. He chose the Fox Valley for his residency and ultimately fell in love with Wisconsin for the genuine community and family friendly values. He is excited to practice full spectrum medicine while developing a relationship with his patients in our community. David joined the Grant Regional team in 2022 and has appointments available in Lancaster, and in Fennimore on Wednesdays. Click the button above to learn more about Dr. Casillas Plazola.

Groundbreaking - March 2023

Grant Regional Health Center hosted a groundbreaking ceremony on Monday, April 3, to mark the beginning of construction of a family practice clinic in Fennimore. The new clinic will bring additional healthcare providers and services to the Fennimore community with anticipated completion by end of 2023.

“This is an exciting day for not only our board, leadership, and staff of Grant Regional Health Center, but also an exciting day for the community of Fennimore and surrounding area. Fennimore is a growing community and we have seen an increase in patients from this area come to our Lancaster clinic. That increase in volume prompted us to consider bringing these important services to your community,” said Dave Smith, President/CEO of Grant Regional Health Center at the ceremony. "Our hospital has had a longstanding commitment to the Fennimore area. We opened a clinic in this community in 1993 – thirty years ago this year. We are excited to renew that commitment today as we break ground on an 8,000 square foot, state-of the-art facility that will provide a full spectrum of care for new and existing patients and their families.

Lydia McClimans, City of Fennimore Community Development Manager, shared on behalf of Fennimore Mayor Ryan Boebel, that he was, “extremely excited about the new facility GRHC has decided to construct in our city. Since the day the council decided to split the lot in our business park to allow for the construction of a dental clinic, he had envisioned how great it would for the community if a healthcare provider would be willing to build on the remaining portion of the lot. He was elated the day he learned about the Fennimore project, as this was exactly the dream he had in his head.” Lydia said, “maybe he could see the future.”

Jason Kenny, Grant Regional Board Member said, “We’d also like to recognize Delta3 for the design and engineering and Midwest Builders of Fennimore who has been selected as the general contractor. With any important initiative that our leadership and board of directors pursue, we first look at the needs of our community. We also agree that timing has to be right and we believe that the time is right to expand and grow to better serve the needs of this area. Our growth includes the addition of new providers -which is also very exciting. A familiar name you might recognize is Dr. Ron White who grew up in Mt. Hope and graduated from River Ridge High School. He has recently signed to join our team and will be one of the providers at this clinic location in addition to Dr. David Casillas Plazola who joined our clinic last fall. We look forward to the months ahead as our team carefully plans and executes the construction, and ultimately, the opening of our clinic. It will be amazing to watch it take shape and incredibly exciting to open the doors later this year.”

The financing group is headed by Peoples State Bank and includes Community First Bank and Royal Bank. In addition, this project will be partially funded by a USDA Impact Grant, that has awarded rural hospitals funding to advance ideas and solutions to solve regional rural health care problems to support the long-term sustainability of rural healthcare services. “This is an important project and USDA Rural Development is happy to be part of its development,” said Julie Lassa, State Director. “Access to healthcare in rural communities is a priority for the Biden-Harris Administration and is critical to the health and well-being of people living and working in small communities across Wisconsin.”
