Your Voice Matters - Community Health Needs Assessment

Grant Regional Health Center has once again partnered with other health care organizations and county health departments have joined forces to conduct a Community Health Needs Assessment (CHNA), a process conducted every three years to identify and prioritize efforts to address the communities’ health needs.

Grant Regional Health Center, Crossing Rivers Health, Gundersen Boscobel Area Hospital and Clinics, Southwest Health Center, and Grant and Crawford County Health Departments are working together to conduct the 2024 CHNA which will help inform each entity’s Community Health Improvement Plan (CHIP).

“The community survey is a key component of the Community Health Needs Assessment process,” says Jeff Kindrai, Grant County Health Department. “Voices from the community, coupled with county and state data, helps us understand where we’re making progress and where we can do more to address community health needs and disparities.”

This survey is intended for individuals who live, work, or provide services in the Southwest Wisconsin counties of Grant, Crawford, Lafayette, and Iowa, as well as the Northeast Iowa counties of Allamakee and Clayton. The survey will be available during the month of May on partner organizations’ websites and social media pages and in printed form.

Following the assessment process, which includes gathering and analyzing relevant public health and hospital data, surveying the community and talking with key stakeholders, the group will identify the top health priorities to be the focus of their efforts in the years of 2025, 2026 and 2027. Each hospital and health department will develop and implement a plan that addresses these priority areas.

“We encourage everyone to share their voice and share this survey opportunity with others,” says Sonya Lenzendorf, Crawford County Health Department. “It is important to get feedback from a wide variety of individuals that represent our communities. This feedback helps determine the programs and services that will be provided to support a healthier future.”
