The Chronic Care Management (CCM) Program involves a team of healthcare professionals working together to help you achieve your best possible health. Patients living with chronic conditions can often feel overwhelmed when trying to manage their health without support.
With Chronic Care Management, you have a team of healthcare professionals working more closely with you, and having regular check-ins to monitor your progress.

- Reduce costly Emergency Department visits.
- Make you more comfortable when you transition from hospital to home.
- Lessen the burden of chronic disease to improve your quality of life.
- Coordinate needed care by healthcare providers and services you may benefit from.

Most patients who chose to participate in the Chronic Care Management (CCM) program notice an improvement in their health and overall wellbeing as a result of the program, including:
- Better control over chronic illnesses like diabetes or high blood pressure.
- Greater understanding of their medications and their effects
- More cost effective healthcare as a result of improved preventative care.
Your Role
You are in charge of your own healthcare. Your primary contact in the CCM program is your Chronic Care Coordinator. Open communication with your Chronic Care Coordinator helps us know how you are doing and addresses any issues that may arise. Your responsibilities as a patient in the CCM program include:
- Completing regular check-ins with your Chronic Care Coordinator.
- Reporting any progress and side effects of current medications on a regular bases.
- Communicating any new symptoms, illness, or concerns you have, right away.
Your Chronic Care Coordinator’s Role
You will have a close relationship with your Chronic Care Coordinator. Your Chronic Care Coordinator will:
- Work closely with you and your healthcare team.
- Be your advisor and educator, helping you understand your medications, diet, activity, and overall lifestyle factors.
- Answer questions that arise and ensure that you have the resources you need.
